Achieving the goals of Integrated Primary and Community Care through a quality lens
Patients as Partners: Achieving the Goals of IPCC Through a Quality Lens, delivered at The Change Foundation’s Meeting of the Minds 2011, Kelly McQuillen presents BC’s experience and progress on their province wide Patients as Partners initiative. The program is a key part of the Integrated Primary and Community Care (IPCC) approach to better address the needs of seniors with chronic health conditions.
Patients and families will be encouraged to:
• participate in their own health care
• participate in decision making about that care
• participate at the level they choose
• participate in quality improvement and health care redesign in ongoing and sustainable ways.
