This chart pack is part of a suite of resources based on recent data on home-care use in Ontario mined by Dr. John Hirdes and his University of Waterloo research team. These resources focus on Ontario seniors living with chronic disease, and those moving from hospital to home and/or community care, pinpointing key challenges they experience, helping to connect dots and/or raising flags for policy makers.
