Integration of Care: Perspectives of Home and Community Providers

One of four technical reports by research consultant Patricia Baranek for The Change Foundation about integration of care from the perspectives of home and community care providers in Ontario. The technical reports provide detailed analysis of the survey responses used to develop our 2011 report, Integration of Care: Perspectives of Home and Community Providers.

This comparative summary report represents the results of all the surveys conducted with:
• regulated health professionals: registered nurses, registered practical nurses, rehabilitation therapists (occupational therapists, physiotherapists), speech language pathologists, dieticians, social workers, respiratory therapists, pharmacists
• personal support workers
• care coordinators: Community Care Access Centre (CCAC) case managers and system navigators, care coordinators in provider organizations, intake coordinators and order processors (medical equipment and supplies).
