Loud and Clear – Seniors and caregivers on navigating Ontario’s system: microsite

This microsite features our first report designed to be navigated and absorbed online. Loud and Clear reflects what seniors with chronic conditions, and their informal caregivers, told us about their experiences navigating healthcare transitions in Ontario. It offers new data and unique insights into what would improve their health, quality of life — and the quality and sustainability of our system.

The report is introduced via video by CEO Cathy Fooks who highlights its messages: Connect all parts of healthcare, especially primary care. Communicate early, often, across the board: provider to patient/family; provider to provider; place to place. Include patients and family in decisions that affect their lives. Don’t let people facing barriers fall behind.
