Breaking New Ground: PATH video profiles partners, promise of co-design

This video highlights PATH’s June 5th launch in Cobourg where The Change Foundation awarded the Northumberland Community Partnership $3 million to improve healthcare transitions and experiences. CEOs Cathy Fooks and Robert Biron outline the purpose and promise of PATH, the first system-wide venture in Ontario to make patients partners in co-designing healthcare.

You’ll also learn why local family caregiver Gayle Einarsson has been involved since the start. “We need patients and caregivers at the table, in the meetings, talking and listening, sharing what they need from the healthcare system,” says Einarsson, a project team leader. She and her parents share their healthcare story, reflecting the realities facing all family members after a stroke hits.
