PATH – Partners Advancing Transitions in Healthcare: Breaking New Ground

In Northumberland, seniors, caregivers, and health providers will work together to co-design services and improve healthcare transitions – a trailblazing $3-million project funded and overseen by The Change Foundation.

This slide deck describes project PATH(Partners Advancing Transitions in Healthcare) in full. Goals and philosophy. Timelines. How Northumberland was chosen as the PATH site. And an introduction to the Northumberland Community Partnership. It was presented at the launch of PANORAMA because the two projects intersect. PANORAMA panelists will provide a wide-angle view of PATH initiatives and progress.

PATH is walking the Foundation’s talk: patient/caregiver input driving redesign that will improve people’s healthcare experiences by addressing a critical problem point: healthcare transitions.
