Facing the Facts: Understanding Ontario’s ALC patients

Fact: The percentage of alternate-level-of-care (ALC) patients with moderate care needs waiting for long-term care has varied by as much as 20% across Ontario’s LHINs. What accounts for this?

That’s one of the vital policy questions raised by this fact sheet, one of a set of ready-to-insert resources with companion, presentation-friendly chart packs with over 30 graphs. The resources are based on recent data on home-care use in Ontario mined by Dr. John Hirdes and his University of Waterloo research team. These resources paint a clearer profile of Ontario seniors living with chronic disease, and those moving from hospital to home and/or community care, pinpointing key challenges that patients experience, helping to connect dots and/or raising flags for policy-makers. This fact sheet zeroes in on the composition and service needs of Ontario’s ALC population, pegged at 16% in 2010.
