Quality ImprovementQuality Improvement: Community and Home-care ServicesThe quality improvement research agenda includes initiatives to improve the continuity of care among health-care sectors and to contribute to evidence-based decision-making home/community care. Key current projects focus on: A Hospital to Home project in partnership with the Ontario Association of Community Care Access Centres (OACCAC) launched January 2008 to map out and improve the interactions and decisions patients/caregivers must make when leaving hospital.

The Centre for Healthcare Quality Improvement at The Change Foundation As of July 1st, 2008, CHQI has joined forces with The Change Foundation. Formely known as the Ontario Health Performance Initiative (OHPI) of the Ministry of Health and Long-term Care, CHQI is now part of The Change Foundation and will operate at arms-length from the provincial government. The initiative was established in 2006 to accelerate quality improvement in Ontario to improve system-level outcomes in areas of provincial strategic priority.
