With the launch of Framing the Experience, the Foundation seeks to find the look and feel of people’s healthcare experiences across Ontario. Photo Contest runs April 1 to July 1.

This video takes you to Cobourg where leaders launch PATH June 5, 2012. A Northumberland couple tell their healthcare story and the family caregiver shares hopes for PATH’s patient-provider partnership.

Northumberland PATH’s resident gerontologist Maryanne Brown will add her knowledge of aging to ground the project’s work. She says the project “goes beyond where healthcare has gone before.”

We are awash in stats, but few relate to patient-caregiver experience or reveal the faces behind the facts. A Foundation board member shares both, and says identifying health service shortcomings from patient/caregiver experiences can prompt system improvements.

Highlights of Change, Annual Report 2009-2010, is a streamlined look at the Foundation’s contributions to creating healthcare services that are connected and coordinated around the expressed needs of its users.
