One of four technical reports by research consultant Patricia Baranek for The Change Foundation about integration of care from the perspectives of home and community care providers in Ontario. The technical reports provide detailed analysis of the survey responses used to develop our 2011 report, Integration of Care: Perspectives of Home and Community Providers. 

This technical report provides detailed results of the surveys conducted with care coordinators:

  • case managers and system navigators with Community Care Access Centres (CCACs)
  • care coordinators in provider organizations
  • intake coordinators and order processors (medical equipment and supplies)
