Path Project Manager Wendy Kolodziejcak on Roll-Out of Path Pilot Phase

Project-Update-by-PATH-Project-Coordinator-Wendy-KolodziejcakThe roll-out of PATH’s pilot phase, now underway, is as multi-faceted as PATH itself. In this audio clip, project manager Wendy Kolodziejcak identifies key elements to be tested during the pilot: the use of mobile tablets to empower patients, facilitate connection, and get “real time” feedback from patients and providers; the Planning Ahead and Aging Well component, with seniors creating their own plans; and PATH’s work on patients’ self-monitoring. Kolodziejcak describes how the roll-out will begin with 25 seniors and move out to 300. “It will yield rich, meaningful data – to help us improve healthcare transitions and really understand what needs to be in place to reduce unnecessary ones.”
