Why did the Change Foundation launch its linchpin Northumberland PATH project? Find out in this quick five-minute video drawing out (a la Dr. Mike Evans) the essential – why, how, so what? – elements. This engaging story outlines how PATH arose from the think tank’s early work, revealing that the direct patient voice was missing in the health care integration research and literature.

Its whiteboard style lays bare the project’s key priorities co-designed by patients, caregivers, providers to improve experience, and ease transitions, including: enabling Aging Well technology to connect care in real-time, My Health Story to capture a person’s health and life story; voluntary peer coaches; and a unique person-centred model of care.

The video outlines early progress, making clear that health care in the community is forever changed post PATH, as patients and caregivers emerge as empowered partners in care.
